THE ODISHA LABOUR WELFARE FUND ACT, 1996 

 Notice of Opening (Form-A) : 

Any factory defined under 2(m) of the factories Act, 1948. A Motor Transport under taking as defined under 2 (g) of the Motor Transport Workers Act, 1961. And any other establishment as defined under 2(8) of the Odisha Shop & Commercial Act, 1956.And includes a society registered under any law in force in the State relating to registration of Societies, and a charitable or other trust, whether registered or not, which carries on any business or trade or any work in connection with or ancillary thereto and which employs, or on any working day during the preceding twelve months employed, twenty or more persons, but does not include an establishment, not being a factory, belonging to or under the control of the Central or any State Government are liable to deposite contribution under Odisha Labour Welfare Fund Act,1996.

Timeline 

Notice of opening under any factory defined under 2(m) of the factories Act, 1948. A Motor Transport under taking as defined under 2 (g) of the Motor Transport Workers Act, 1961. And any other establishment as defined under 2(8) of the Odisha Shop & Commercial Act, 1956, is provided within 15 working days from the date of submission of application. The department can however raise queries on the application within 7 working days of receipt of application. Timeline will resume on receipt of query compliance. 

Notice of Change (FORM-B) : 

Every employer shall within thirty days of any change taking place in regard to any information furnished in form- 'A', serve a notice of such change to the Welfare 
Commissioner, in Form B: Provided that in case a particular type of information changes frequently in any establishment, the notice of such change may, with the approval of the Welfare Commissioner, be served within fifteen days from the closing of each half year ending the 30th June and the 31st December.

Notice of closure (FORM-C) : 

Where an employer intends to close down his business for good and sufficient reasons, he shall serve a notice of such intended closure in Form C on the Welfare Commissioner at least, sixty days before the date on which the intended closure is effective: Provided that any such closure shall be in accordance with the provisions of law

Statement regarding Contribution (FORM-F) : 

Employees working under any factory defined under 2(m) of the factories Act, 1948. A Motor Transport under taking as defined under 2 (g) of the Motor Transport Workers Act, 1961. And any other establishment as defined under 2(8) of the Odisha Shop & Commercial Act, 1956 are the eligible beneficiaries under the Act.
